*A medical release & permission form must be on file in the church office before a student can attend any off-campus event/gathering/trip that is sponsored by AMPED Student Ministries.

UPDATE:
When you open the 2009 Medial Release Form in Adobe, you will be able to type in the highlighted fields all the non-signature necessary fields and then print the form, get it signed and notarized, and then turn it into the office.
